Transition of person-centred, interprofessional care
There is limited time for interprofessional health care, because appointments with single professions are focused. Time and space to meet and exchange between service users and providers for person-centred, interprofessionl shared-decision are needed: for example in primary care or in transition from rehabilitation to first-line practices. Which digital solutions help? What international solutions could be transferred to other countries' health systems? How efficient could a virtual "specialist hour" or "Person-centred Hackathon" be implemented (which are ideas that result from the INPRO project consortium)?
